We are searching for a full time outpatient case manager to join our team in Mansfield.
The ideal candidate for this exciting career will have integrity and a passion for helping others.
Working closely with the clinical supervisor, you will be processing intakes, discharges, scheduling requested evaluations, and may provide therapy services.
With this position, you will need to be very organized and efficient with all paperwork.
What Do I Need? Education and Experience Bachelor's Degree required in human services or related field AND Two years of experience in the field of human services Other Requirements You will maintain a valid driver's license and have the ability to travel.
You will maintain all required clearances.
You will have the ability to use typical office equipment.
You will have the ability to work as a part of a team and independently.
You will have excellent organizational, interpersonal, and communication skills.
You will follow all HIPAA privacy regulations.
What Will I Do? You will process incoming clients for the appropriately assigned Outpatient program You will be in contact with the clients' families and therapists to ensure everyone is up to date with information.
You will schedule assessments after obtaining authorization.
You will facilitate interagency meetings and handle corresponding paperwork.
You will prepare and maintain your client files and participate in all meetings within the agency pertaining to your client You will work in a professional manner and cooperate with CONCERN staff and other agencies.
You will take responsibility for all formal reports and contacts with all internal and external people involved.
Hours of Work You can expect to work in an office setting from 8:
30-4:
30 or 9:
00-5:
00 depending on scheduling.
